The Programming Committee has pulled together a diverse panel of experts to talk about Population Health across the continuum.  Topics include the big picture of Population Health versus the smaller efforts to manage chronic illness, the interface of the EHR and regional health information and big data, the primary care network to the hospital and post-acute care continuum and finally behavioral health and substance use. We are pleased that ACHE Board of Governors, Brig Gen James J. “Jay” Burks, FACHE will be joining us for opening remarks. The agenda and panels are described below.
